Please ask for original receipt from the agent, remember to get them sign
and submit together with the following documents after your return from
home town. Let me know if you the city you are flying to is not in the
list.

1) Requset form

2) Employment Details form & Personal Particular form

3) Appointment letter and acceptance letter.

4) Claim form

(See attached file: Claim-AirfareGtg(14092004).doc)(For those who
purchase their own airticket, reimbursement based on MOE quotation and the
airfare paid by the scholars, whichever is lower)
(Remember to sign the form and attached the original receipt)

5) Copy of passport showing the the Entry and Exit date into/from your
hometown.

6) The counterfoil of the airticket.

As an example, JiNan is not listed in the form, but I bought a ticket to jinan and claimed. so u can also try.

Just drop her an email, and she will send u the forms required.
