Below is what I saw from Singapore General Hospital's website.

Referrals
Primary healthcare doctors and those from polyclinics and other hospitals refer patients who require specialist care to our Specialist Outpatient Clinics.
There are two types of referral:
General Referrals
Your would be on a general referral and seen as a subsidised patient if you are a Singapore resident referred by a government doctor/hospital without reference to a specialist by name.
Private Referrals
You would be on a private referral and seen as a private patient if you are referred to a specialist by name by a government doctor/hospital or referred by a private doctor/hospital, or if you are a foreigner.
